Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW PROTECTED HEALTH INFORMATION (PHI) ABOUT OUR PATIENTS MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW CAREFULLY.

Our Duties:
MedLabs is committed to protecting the privacy of your protected health information (PHI). It is our responsibility and legal duty to make sure PHI remains confidential in accordance to the Health Insurance Portability Accountability Act of 1996 (HIPAA).

Conditions of PHI Use:
MedLabs primarily uses your PHI in patient diagnosis, payment, treatment and other clinical operations that are legally permitted. Uses and disclosures vary according to state laws under the Clinical Laboratory Improvement Act (CLIA). Below is a list of different uses your PHI may fall into. All other uses may not be listed specifically, but fall into one or more of the categories listed below.

· Clinical or Diagnostic Purposes
MedLabs aids healthcare professionals in the diagnosis and treatment of a patient through various laboratory procedures. In order to result any test, we must first obtain a patients PHI along with a sample for testing. Your information will only be disclosed in a testing environment to authorized personnel at MedLabs.

· Billing
MedLabs may use your PHI along with your insurance(s) on file for billing purposes in order to receive payment for the provided services. If the patient has a guarantor, invoices may also be sent to whomever is the selected guarantor for that patient (some examples of a guarantor are, but not limited to; a parent, spouse, or domestic partner). Invoices may include PHI and information involving performed services by the laboratory and doctors information.

· Business Associates
Business associates that work with MedLabs in order to provide services also have access to our patient’s PHI. All parties are required to maintain privacy and comply with MedLab’s company policy, HIPAA and state regulations while handling our patient’s PHI.


Some exceptions may be made as required by law.

Your Patient Rights:
You have the right to access your PHI and lab results. You may do so by completing this form.

You have the right to obtain a copy of this Notice upon request.

To exercise your rights you may write or send an email to us with your specific request, including requesting a form to complete to obtain a copy of your test results.

Note:
We reserve the right to amend the terms of this Notice to reflect changes in our privacy practices, and to make the new terms and practices applicable to all PHI that we maintain about you, including PHI created or received prior to the effective date of the Notice revision. Our Notice is displayed on our website and a copy is available upon request.
